If you would like to join any of our THM Herbal Programs please fill in your details in this Application Form below and submit the application fee. *Applications are reviewed on an ongoing basis. Applicants are strongly urged to submit application materials as early as possible due to limited class size. Incomplete applications will not be considered. *An interview with the Program Director may be required. All materials and information filed during this application process become part of your permanent, confidential record and are not returnable.
